SMZ-TMP DS (sulfamethoxazole-trimethoprim) 8oo 7 days			Sinus infection	Erythema multiforme Sore throat/throat irritation Mouth sores	This was the first time I have ever taken this medication and I reacted badly almost immediately. I had a sinus infection with a mild sore throat (which I assumed was due to drainage) and was prescribed SMP-TMZ. That very day my sore throat became very painful and I began noticing a burning sensation in my mouth. At first I thought it was due to my infection, but then my entire tounge turned white and I had what looked like red welts on my tounge. My whole mouth was on fire and soon my tounge and throat were swelling. I could still breath, but the swelling was definitely noticeable. It's been 4 days since I've been able to eat anything solid (and still no sign of relief). My meals consist of room temp. chicken noodle soup and luke warm tea. I was popping advil like crazy and going through a bag of cough drops every day. I went to the ER with my conditions and was told by the Dr that there was no swelling, I had a throat infection (from what, he could not tell me) and he didn't know what was wrong with my mouth. He then said there was nothing he could do and sent me home saying to take tylenol and use cough drops (hello! I already tried that!) When I commented that my tounge was so swollen I couldn't close my teeth around it and was finding it difficult to form words properly he said "well how am I supposed to know what your tounge looked like before this?"! The nurse, however was fantastic. She thought I might have Stomatitis and brought in another nurse that had been there for several years. They both agreed that it appeared this is what I had and said it can be related to allergic reactions. I went home, looked it up on the internet, and sure enough, it sounds exactly like what I have and is related to allergic reactions. I understand that doctors won't know you're allergic to something if you've never used it, but these side effects should be printed up with the others. Effexor XR (venlafaxine) 150 1 days			Intestine problems Muscle twitching Body pain Involuntary eye movements Headache	Nystagmus	I am just now finding out all the side effects to this drug. I am slowly coming off of Effexor. I tried to come off Effexor by supplementing a different drug Savela but the rapid eye movement became worse. I have lost several days of work. Over the past few years I have spent so much money trying to find out what is wrong with me. I had no sexual libido (inability to achieve an orgasm), muscle spasms, pain in every part of my body, sever fatigue, migraines. I have taken so much medications to counter the symptoms I was just going around and around in a circle. I truly hope that I am on the road to recovery. amoxicillin 500 mg 20 days			Sinus problems Ear inflammation Sinus infection Ear pain/ear ache Ear infection	Temporary nausea	I have been prescribed amox. multiple times for sinus infections and have rarely had any side effects if any unlike other antibiotics. There is no place on this form to put in tolerance where you enter withdrawal because the drug is no longer working that is what happend to me and no doctor could figure it out so I sufferred for years till I broke. Mucinex D Max Strength (guaifenesin-pseudoephedrine) 1 tablet 1 days			Headache located above the eyes	Insomnia and other sleep problems Trouble falling asleep	I never take medications other than aspirin or Tylenol. I had symptoms that seemed like allergies so I took one single tablet of Mucinex D. I took no caffeine or any other substance.
